Our referral forms
You may contact us directly as a “self-referral” or referrals can be made through your solicitor, CAFCASS, probation officer or social worker.
The links below will open the forms. You can complete the forms online, but if you would prefer to print the form and complete it by hand, the form can be downloaded. Either post the form to us, or scan or take a photo of the form on your phone and send by email. See the Contact us page for details of where to send the forms.
Referral forms
Each parent should complete a referral form. The two forms are different. One is intended for completion by the resident parent (parent living with their child/children), and the other form by the non-resident parent (the parent wanting to have family time with their child/children).
If need be, your social worker or another professional can help with your application.
